We ask that all players and parents review and return a photo release form.

MINOR (CHILD) PHOTO RELEASE FORM

I grant Francis Howell Hockey Club (FHHC) my permission to display photographs, drawings, and other images of my child for legal use to include, but not limited to, the FHHC website, social media sites, illustration, advertising, and publicity use.

Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
